Allows customers specify quantity on subscription upgrade for Graduated pricing model

Add a field to specify quantity for customers when they upgrade subscription via Customer Portal for Graduated pricing model. Right now only admins can specify quantity in the LemonSqueezy dashboard when upgrade the subscription.
